Individual provider subscriptions. Effective July 10, 2026. Version 1.
This agreement applies to individual AllergenMaps provider subscriptions purchased through this site before July 28, 2026. Individual subscriptions are no longer sold; existing ones continue under this agreement. Pharmacy, group, and enterprise accounts provisioned through access codes or a signed agreement are governed by that agreement rather than this one.
Your subscription grants you a personal, non-exclusive, non-transferable license to use the AllergenMaps clinical tools in your own professional practice. The license covers one named individual: the person who created the account and accepted this agreement.
Your sign-in credentials identify you personally. You agree not to share them with any other person, including colleagues and staff, and to take reasonable care to keep them confidential. Each additional user in a practice requires their own subscription or a group plan.
Your account may be signed in on one device at a time. Signing in on a new device ends your session on the previous device. Attempting to circumvent this limit, including by rapidly alternating between devices, is a material breach of this agreement.
To enforce this agreement, we log sign-in events, device registrations, network addresses, and request volumes, and we review usage patterns consistent with credential sharing. This monitoring is described in the Privacy Policy.
If we determine that an individual account is being shared, we may suspend or terminate access without refund. We will ordinarily contact the account holder first and offer an appropriate group plan.
If more than one person at your practice needs access, contact support@allergenmaps.org for group licensing. Group accounts provide a named sign-in for each staff member.
This agreement supplements the Terms of Use. Where the two conflict regarding account use and credential sharing, this agreement controls.
We may update this agreement. Material changes will be presented for acceptance at your next sign-in or renewal.
You accept this agreement by checking the acceptance box during signup or checkout. Your acceptance is recorded with a timestamp on your account.
